Why Documentation Matters More Than It Seems

Documentation is often treated as paperwork to get through, but it's actually the record that protects the client, protects you, and determines whether care gets paid for and continued. If it isn't written down, from a regulatory and legal standpoint, it didn't happen -- even if you did the work perfectly. Good documentation also communicates to the next caregiver, the nurse, or the family what happened during your visit, which matters most when something changes and someone needs to piece together the timeline.
